POSITION TITLE: Registered Nurse (RN) P/T .4 FTE with Weekend/Holiday Rotation
HOURS: As discussed with Director of Health Care Services and Executive Director. Hours may vary according to caseload.
RESPONSIBLE TO: Director of Health Care Services.

JOB SUMMARY: The role of the Registered Nurse (RN) is to ensure that the health and safety of all tenants are met through on-going on site visits with the tenants and staff. The RN will ensure that staff is trained in health related policies and procedures which are current and up to date. The RN will also ensure that the facility is in compliance with current regulations and standards. Must be able to relate professionally with tenants, staff and family members, display a positive work attitude and be able to work as a team with other staff members. Assists with the marketing of the program. Must comply with HIPPA regulations. Must maintain tenant confidentiality.


• Graduate of an accredited school of nursing.
• Must be registered and licensed with the State of Minnesota as a RN, and must maintain licensure.
• Must be CPR certified.
• Must have a valid driver’s license.


• Completes initial screening assessment of potential tenants to assure that their needs will be appropriately met.
• Completes a full health assessment to home care client within 5 days of initiating home care services and prior to initiation of delegated nursing services.
• Assures tenants maintain residency criteria. Reassess and recommend alternative placement if necessary. Meets with tenant, responsible party and Executive Director to discuss condition changes leading up to alternative placement.
• Implements service agreements with all new tenants. Reviews and revises service plan as needed and a minimum of 1x per year.
• Assures tenants have up to date care plans.
• Monitors and maintains tenant charts and clinical progress notes.
• Communicates to physicians and other personnel changes in tenant’s needs or conditions.
• Communicates regularly with tenant’s responsible party.
• Facilitates coordination of tenant services with other service providers.
• Is responsible for ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications and/or medical conditions.
• Assures all physicians orders are signed and implemented with-in 24 hours.
• Fills weekly pillboxes and sets up MOTs for medication administration by unlicensed staff.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Assures new medication sheets are in place by the first of each month or according to MOT cycle.
• Reviews each tenant’s medication sheet monthly and assures accuracy and appropriate signatures.
• Communicates in RN log any information that staff need to be aware of.
• Documents disposal of medication per procedure.
• Performs supervisory visits 14 days after admission to Home Care and at least every 90 days thereafter.
• Works as a team player with all members of the tenant’s care team.
• Provides health and medication training to all new employees as well as continued education for all staff and documents such training to assure compliance with state, federal and local regulations.
• Supervises medication practices, nursing procedures, and medical equipment being used in the building.
• Assures that staff clearly understands and follow all tenant care procedures.
• Assists Director of Health Services with staff evaluations, coaching and disciplinary actions related to improper care giving techniques. Documents incidents appropriately.
• Monitors staff for accuracy of tenant blood pressures, weights and pulse as requested.
• Shares tenant and family concerns with pertinent staff if appropriate.
• Assists in the development of health related policies and procedures and implementation of them.
• Closes out tenant’s clinical file, including storage, retention and destruction schedules upon discharge.
• Attends monthly staff meetings.
• Works with manager to assure emergency procedures are in place.
• Is flexible and willing to expand and adapt to changes in the AL industry.
• Promotes a positive, team approach and a healthy work environment.
• Is available for on-call medical questions and/or emergencies via phone.
• Assures compliance with regulations and is available for pertinent licensure surveys.
• Other related duties as assigned.
• Provides support, coaching, redirection and supervision of staff.
• Provide ongoing training and education as needed.


• Must be able to move throughout the work day and throughout the building.
• Must have adequate vision and hearing to meet the requirements of this position.
• Must be able to cope with the mental and emotional stress of the position.

The employee must regularly lift and/or move up to 25 pounds, occasionally lift and/or move supplies, equipment, and materials weighing approximately 35 pounds.


• Exposed to body fluids, infection, odors and behavior of tenants.
