Organization Summary:

Community Involvement Program is a non-profit organization dedicated to serving individuals with disabilities throughout Minnesota.


Primary Duties:

Essential Responsibilities:
· Responsible for providing care, interventions and/or services as specified in the written care plan or individualized service plan.
· Work with individuals living in CIP Shared Housing.
· Assists the participant with activities of daily living, including personal hygiene, assistance with grocery shopping, food preparation, and budgeting as needed.
· Conflict management between housemates.
· Conducts self in a professional manner in all work-related functions.
· Record visits in Apricot in a timely and professional manner.
· Assist with the updating of participant’s charts.
· Completing 12 hours of in-service training per year.
· Meet with supervisor and communicate any changes with case load or schedule.
· Turn in time cards and chart notes every 2 weeks to supervisor.
· Reports to supervisor any significant changes in participant condition or family situation.
· Recognizes and utilizes opportunities to reinforce health teaching of participant and family.
· Promoting recovery and instill hope in the lives of the participants.
· Adheres to Universal Precautions in all practices and duties.


Experience:

Credentials/Qualifications:

· Must have a valid driver’s license and clean driving record
· Home health aide certification or ability to pass certification testing or BA in the social services or related field.
· Must maintain ongoing in-service requirements
· Experience working with adults with disabilities.
· CPR/First Aid certification
· Must pass a Department of Human Services background study.


Additional Info:

Work Environment:
The employee in this position typically works in the community in participant’s homes or apartments. Candidates will be expected to work generally Monday – Friday, 5 - 9pm, with some flexibility based on program needs. Part-time shifts are also available.
