Three Links is a progressive health care provider for older adults in the Northfield area. Located just south of the Twin Cities of Minneapolis and St. Paul, the campus includes Skilled Nursing, Respite Care, Memory Care, End of Life Care, Assisted Living, Independent Living, as well as home health services to the greater community.

The Licensed Practical Nurse (LPN)—Home Care is accountable for client care according to professional standards and established policies and procedures. This position is responsible for setting up medications and treatments according to Physician's orders and for accurate documentation of care and maintenance of medical records. The role of the LPN - Home Health Nurse is to ensure that the health and safety of all clients are met through assessments and reassessments of the clients, development and implementation of appropriate service plans and plans of care, direct patient care, monitoring of clients' services, supervision of home care staff and on-going training of home care staff and determining they are competent prior to delegating tasks to them.
The Home Care Licensed Practical Nurse will ensure that staff are trained in nursing procedures and policies which are current and up to date. The LPN will also ensure that the home care services are in compliance with current regulations and standards. Must be able to relate professionally with clients, staff and family members, display a positive work attitude and be able to work as a team with other staff members.
NOTE: Home Care Link is open Monday through Friday 8:00-4:30.

• Graduate of a School of Nursing.
• Must be registered and licensed with the State of Minnesota as a Practical Nurse and must maintain licensure.
• Must be CPR certified and Heimlich maneuver.
• Must have a valid driver's license.

Experience in home health, geriatric, or med-surg nursing is preferred. This position requires an understanding of the fundamentals of the nursing practice and the ability to apply them to the geriatric and home care based setting. Must have a willingness to assist with direct care activities as necessary. Patience, tact, respect, courtesy and kindness are required when dealing with clients, visitors, and coworkers. This position requires the ability to treat client information with confidentiality. Clear verbal communication in English and by electronic medical records, telephone and legible writing is necessary. Must comply with facility dress code.

• Medication and treatment set up weekly.
• Emergency intervention as needed.
• Monitor client vital signs, skin, comfort and ADLs weekly.
• Completes client charting as assigned to accurately reflect clients' care needs.
• Assist HHAs with client cares if needed, especially clients with acute illness, dying or difficult behaviors.
• Physician phone calls, transcription of orders, ordering medications from pharmacies.
• Update client care plans as needed or directed by Home Health Link Director.
• Review, update and start new client medication administration sheets monthly.
• Completion of Incident Reports as needed and assist the Home Health Link Director with follow up after tenant/employee incidents.
• Direct patient care
• Other duties as assigned
